Medtronic CareLink

Web-based program to upload information from Medtronic insulin pumps and continuous glucose monitors (CGMs). For patients with the Minimed 770G and up systems can download a smartphone app that will sync the insulin pump/CGM data with Carelink (iOS users can download the app here and Android users can download the app here).

InPen (by Medtronic)

This is a “smart” insulin pen that helps with calculating insulin doses and keeping track of when and how much insulin is given. The pen device connects to an app on a smartphone. For Apple iOS users the app can be found here. For Android users the app can be found here.

Omnipod insulin pump system (by Insulet)

This is a tubeless, wireless insulin management system. These insulin pumps can be uploaded to the Omnipod Cloud using Glooko. Omnipod users can manage their Omnipod account using the PodderCentral app (for iOS users and for Android users). For Omnipod DASH users, the device can sync with an iOS smartphone app called Omnipod DISPLAY app (currently only available for iPhone and can be downloaded here). This page contains links to all available Omnipod smartphone apps.

***1/28/2022 the Omnipod 5 insulin pump/CGM Automated Insulin Delivery system was FDA approved. This uses the tubeless Omnipod insulin pods and the Dexcom G6 CGM and the insulin infusion is adjusted using information from the Dexcom. You can read more information about this here.

Glooko

Website for uploading data from Omnipod insulin pumps and other devices (if your doctor subscribes to the service). The Glooko mobile app can be downloaded here for iOS users and here for Android users. You can login to the website here. Once you create an account, you will be able to upload the Glooko Uploader software.

t:connect

This is a web-based diabetes management application for the Tandem insulin pumps. You can login to the t:connect system here. The t:connect Mobile app allows users to view the insulin pump information on a smartphone (iOS app can be downloaded here, Android app can be downloaded here). Tandem pump users can also download pump software updates from this page. For prospective Tandem t:slim pump users, the t:simulator app can be downloaded here for iOS users and here for Android users.

Dexcom

This is a continuous glucose monitor that is changed every 10 days. To use the Dexcom with a mobile device, you will need to download the Dexcom G6 Mobile app on the Apple App Store or on Google Play. To share your data with your doctor, you will also need to install the Dexcom Clarity app on your mobile device (on the Apple App Store or on Google Play) and/or upload your device to Dexcom Clarity on your computer. If you wish to let others see your Dexcom data, they will need to download the Dexcom Follow app on their mobile device (Apple App Store or Google Play).

Eversense

Eversense is a continuous glucose monitoring (CGM) system that monitors glucose levels continuously for up to 90 days via an under-the-skin sensor, a removable and rechargeable smart transmitter, and a convenient app for real-time diabetes monitoring and management. The Eversense CGM app is compatible with the devices listed here and iOS users can download this here, Android users can download this app here. If the Eversense user would like to invite others to view their CGM data, the follower can download the Eversense NOW app (iOS users can download this app here, Android users can download the app here).

Freestyle Libre

The Freestyle Libre 14-day and Freestyle Libre 2 systems are flash/continuous glucose monitoring systems. LibreView is a website for uploading data from the Freestyle Libre reader or from the Freestyle LibreLink app. To use your mobile device with the Freestyle Libre 14-day sensors, you will need to download the Freestyle LibreLink app from the Apple App Store or Google Play. To use your mobile device with the Freestyle Libre 2 sensors, you will need to download the FreeStyle Libre 2 app from the Apple App Store or from Google Play.

If you wish to let others see your Freestyle Libre data, they will need to download the Freestyle LibreLink app on their mobile device from the Apple App Store or Google Play.

To connect your Libre device with Dr. Van Diest log into your LibreView account online or using your phone app and use the practice ID drvandiest (see guide).

Bigfoot Biomedical

The Bigfoot Unity™ Diabetes Management System consists of smart pen caps that incorporate integrated continuous glucose monitor (iCGM) data from the Freestyle LIbre 2 and health care provider instructions to provide insulin dose recommendations for people with diabetes who use multiple daily injections (MDI) of insulin. This was approved in May 2021 and when available, the app links will be added here.
