Comprehensive Care Subscription
Enrollment Fee $100
This is a one-time fee at the time of enrollment.
Monthly Membership $100
This package includes in-person or telemedicine appointment, medical care coordination, referrals, device support, and prescription management. It also includes immediate access via text and email. A six month minimum commitment required.
Fee For Service (a la carte)
New Patient Consult $300
New patient consultation in office or via telemedicine.
Follow-up Visit $175
Follow-up visit in office or via telemedicine for an established patient.
